Sponsor Confirmation Form

This form is the sponsor portion of the Candidate Application, and is used for the sponsor to confirm support of the candidate before, during and after the weekend. Sponsors should complete a separate Sponsor Application Form for each candidate. Sponsors are listed as the primary contact on a Candidate Registration Form, and will receive email notification of their candidate's application. Sponsors can also access their Candidate's Registration Form through their own Church Center account. After completing this Sponsor Confirmation Form, the sponsor is responsible for either paying (or arranging payment of) the candidate weekend fee. Candidate fees are $210 ($25 deposit) and can be paid online or by cash/check: * For Space City Tres Dias members, go to your "My Registrations" in Church Center or follow the link in your email confirmation * For non-SCTD members, go to "Pay Online => Candidate Fees" at www.spacecitytd.org * For checks, mail to Space City Tres Dias, Attn: Pre-Weekend Committee, 945 McKinney St #19517, Houston, Texas 77002.

Sponsor Confirmation of Candidate Eligibility *

It is the policy of Space City Tres Dias that Candidates must meet three eligibility requirements: 1) Must be a Christian, 2) Must be at least 21 years of age at the time of the weekend, and 3) If a married female, the husband must attend the weekend first.
